One of my clients was in the market for a questionnaire for her kiddos teachers.  She was looking for a form that would help her get to know the teachers a bit better.  We took all the questions from a form my son received last year from his teacher and jazzed it up a bit.  If you have read my blog or followed me on social media, you know how I feel about, "brown paper packages tied up with string", so this printable was a fun one to create!  Hope you will download and print it to discover your teacher's favorite things too!
